Eye Problems from ADHD Medication

If you, your child or someone you know takes ADHD medication it is important to understand the possible side effects that might impact eyes, cause eye problems and alter vision. The most common prescription medication we see children and even adults taking for treatment of attention deficit hyperactivity disorder (ADHD) is methylphenidate which has a number of trade names such as Ritalin, Concerta, Adderall, Methylin and Daytrana. While the common side effects of loss of appetite, nervousness and difficulty sleeping are easily recognized, researchers reporting in the Journal of the American Association for Pediatric Ophthalmology and Strabismus raised some concern that this treatment may be associated with increased risk of angle closure glaucoma and a disturbance of eye refraction and optical prescriptions. They initiated a study to investigate the effects of methylphenidate treatment on refraction, intraocular pressure (IOP), and the anterior chamber in children with ADHD. This was a pilot study where children diagnosed with ADHD were examined before the start of methylphenidate treatment and again 3 and 9 months after the start of treatment. Their examinations included an eye exam with ordinary as well as cycloplegic refraction-one performed with eye drops and high resolution imaging study of the anterior chamber of the eye where the delicate structures related to glaucoma could be viewed and measured.

ADHD Medication Eye Study Conclusions
The researchers found that methylphenidate does not seem to affect refraction, or optical prescription in most children with ADHD. But, after 9 months of treatment there was a reduction in the eye anterior chamber depth, which has been described as a powerful predictor of angle closure glaucoma. As this was a pilot study more work is needed to really understand any increased risk.

What You Need to Know and Do
If you, your child or someone you know is taking methylphenidate of any type for ADHD, it would be worthwhile scheduling a routine eye examination so that we can measure the refraction, the intraocular pressure (IOP) and anterior chamber angle and depth. We do this regularly during your eye exam. BE SURE TO TELL US YOU ARE TAKING METHYLPHENIDATE. If your eye exam is normal we will most likely ask you to have a repeat exam in a year. BUT, if at any time there is a change in your vision, pain, redness, glare or light sensitivity we want you to call right away and schedule an immediate appointment for that day.

Predictors of Macular Degeneration Treatment Results

Predictors of Macular Degeneration (AMD) Treatment Results
Age Related Macular Degeneration (AMD) continues to pose the potential for causing vision loss in those over 50 and especially seniors. Today, with early diagnosis and treatment we can often preserve eye health and vision. When we examine you and find evidence of AMD we might recommend treatment with injections of medications such as Lucentis® at carefully prescribed intervals. We also will take the time necessary to answer your questions, in particular regarding what your visual results might be from having the carefully prescribed series of Lucentis® injections. By using information from studies that help us predict the 12 month treatment response to Lucentis® in patients with wet macular degeneration we can make some approximate predictions for your visual prognosis.

Initial Examination Predictors of Results
From these studies we can predict that those patients with lower Best Corrected Visual Acuity (BCVA), of a younger age, with smaller areas of new blood vessel growth and leakage under the retina are most likely to have a good chance of gaining 3 lines of BCVA after 12 months of treatment. This means that should we make the diagnosis of wet macular degeneration and prescribe the Lucentis® injection treatments, you should be prepared to be committed to at least a 12 month treatment plan with appointment scheduling we will recommend based on your individual situation. By following through with this set of recommendation we have the best chance of preserving and improving your vision.

Eye Injuries in Children from Dogs

Eye Injury in Children
Eye injury remains the leading cause of monocular blindness in children, despite the fact that 90% of injuries are potentially preventable. Children playing with pets require supervision and education on how to treat pets. Children interact with animals in a variety of situations, and the associated dangers may be underestimated by parents and those supervising children. Eye injuries in children from pets can often be serious and cause severe eye problems-but they too are preventable. It is not uncommon for children’s dog bite injuries to include corneal abrasions, corneal and sclera lacerations or punctures of the eye itself, all of which at a minimum require aggressive treatment and in many instances repeated surgery. Further, animals are capable of causing damage to the delicate tissue of the eyelids and surrounding tissue that can be cosmetically and visually devastating-also requiring multiple surgeries.

Typically, when very small children-age 4 and younger-are bitten by dogs, eye injuries occur about 15% of the time. The dog is usually one the child is familiar with, and second attacks by the same dog are likely to cause more serious injury. It is recommended that any dog that bites a child be removed from the household. With caution, education and supervision pets and children can be quite compatible but does require some care.

Skipping AMD Treatment Affects Vision

About AMD Treatment
For patients with Age Related Macular Degeneration (AMD) who are being treated with VEGF Inhibitor injections such as Lucentis® and Eylea®, even though it might seem like a great deal of trouble and effort to be totally consistent and compliant with all scheduled appointments-IT IS CRITICAL! The need for consecutive, carefully timed injections according to clinically tested protocol is what helps us preserve your vision and avoid the untoward, potentially catastrophic effects of vision loss from Age related Macular Degeneration (AMD). Researchers reporting in the journal Eye actually studied what happens to the vision of patients who skip even one injection visit. In France, summer vacations are taken very seriously and many French patients leave their doctor’s care for 6-8 weeks as they travel around Europe for the summer. The French eye specialists found that patients who skipped even a single injection during their holiday period were prone to a decrease in vision, formation of cysts and fluid in the macula of the eye being treated. Angle Closure Glaucoma with Certain Medications

About Angle Closure Glaucoma
Angle Closure Glaucoma is a type of glaucoma caused by a blockage or complete closure of the drainage structure of the eye called the trabecular meshwork. The trabecular meshwork is actually a fine filter, and if it is blocked or obstructed by any alteration in the size or shape of the surrounding structures, or by change in the size or shape of the tissue itself, it will cause the intraocular pressure (IOP) to elevate. In instances where the meshwork becomes blocked abruptly, it will cause a sudden rise in the intraocular pressure (IOP), resulting in Acute Angle Closure Glaucoma. Acute Angle Closure Glaucoma is characterized by this sudden rise in pressure which will cause pain, redness, light sensitivity, colored haloes around lights, nausea or vomiting, and blurred vision, and if left untreated permanent loss of vision.

Medications Can Cause Angle Closure Glaucoma
In patients who may already be at risk for Angle Closure Glaucoma because they have certain tissue and anatomical features inside their eyes, certain medications can significantly increase their risk. Two medications that are worth mentioning are the antidepressants Wellbutrin (Buproprion) and Topamax® (Topiramate). While both of these are often used to treat mild to moderate depression, they are also used to help patients stop smoking! Researchers reporting a study in Archives of Ophthalmology found that the risk of angle-closure glaucoma in patients younger than 50 years was twice as high in patients taking Wellbutrin and more than 5 times higher in patients taking Topamax®.

My Cataract Story
